HIPAA notice of privacy practices

Vera is committed to protecting your Protected Health Information (PHI). We use and disclose your information only to provide your care, coordinate with the specialists you choose, run our practice, and where the law requires.

We will never sell your health information. We use the minimum necessary information for any given purpose, and we tell you when our practices change.

  • Your medical history, visit notes, and care plans
  • Insurance and billing details, where applicable
  • Secure messages with your care team

How it's protected

Records are encrypted in transit and at rest, access is limited to your care team and the staff supporting them, and every access is logged.

Your rights

  • Access and obtain a copy of your records
  • Request corrections to your information
  • Ask for a list of certain disclosures we've made
  • Request confidential communications
  • File a complaint without retaliation

Membership terms

Membership covers your primary care relationship and works alongside most insurance. It is month-to-month, cancellable any time, and is not itself insurance. Specialist care, labs, and prescriptions are billed through your existing coverage.
